Pediatric Palliative Care

The goal of Pediatric Palliative Care is to have a trans-disciplinary team or Pediatric Advanced Care Team (PACT), deployed at the time of diagnosis of a life-limiting or life-threatening condition.  The focus is to provide a coordinated care plan inclusive of the patient, family and their primary healthcare team in: optimizing pain and symptom management, addressing psychosocial and spiritual needs, coordinating inpatient, outpatient, home care and goal-setting for the patient and family.  This will minimize fear and suffering for both the child and family, improve the patient experience and provide the necessary education for the patient and family to make quality of life decisions.
