Mission Community Caramedics

Mission Community Caramedics will provide in-home visits by trained paramedics to support patients in managing their health at home - to facilitate their best care and peace of mind.  There's a real advantage to seeing the patient in the home.  From assessing the patient's resources and abilities to performing field tests such as laboratory work, and offering advice for home care, wound management, walking assistance, falls prevention, medication review and nutrition, Caramedics can do it all.  This network of support will be integrated to the patient's electronic medical record (EMR) to facilitate communication among providers and enable better patient care and outcomes.
